Eligibility:
Employees working 20 hours or more per week and a minimum of ten months per year are eligible.
Participation is optional.

This plan is underwritten by Lincoln Financial.

Voluntary life insurance benefits may be purchased (or canceled) for you and/or your dependents. If you would like to enroll or make a change, complete the Voluntary Life Enrollment Form.
If you currently have voluntary life insurance, you may receive a separate letter regarding options to increase your coverage.
If you are currently enrolled, you can view your current level of coverage at Lincoln Financial
- choose login/register > individual > register > employee benefits
View the Certificate of Insurance from Lincoln Financial to access your summary plan description.
Complete this Benefits Designation Form if you need to change your primary and/or contingent beneficiaries.
